File the motion, a notice of hearing (leaving the date for the motion to be heard blank), proof of service and a copy of the motion marked "Judge's copy".

The Court will schedule the motion date upon receiving the paperwork and will send notices to the appropriate parties. If you would like the Court to contact you prior to scheduling the motion to confirm a motion date, please request this in writing in your cover letter.

Effective October 1, 2003, a $20 motion fee is required on all motions. If paying by check or money order, make check or money order payable to 52/3 District Court.
